Actions and Gathering Feedback on Data

I am trying to create an action for my dashboard to gather feedback from users and write it back to the BigQuery DB.

For example, I have the sales dashboard of a chain and each user as a store manager has to write a feedback on their store’s sales. 

Does anyone know what is the best approach to make this work?
