Action link to form of another app with pre-populated field

In my action I would create a link to a form of another app that for the field LINK contains the current url of the app where the action is clicked.

How could I proceed?

The LINKTOFORM() function includes an optional parameter for APP to allow navigation to the form in a different app.  This function also provides the ability to pre-populate fields on the Form view.
