App with sub-menu

Hello, 

I have created a survey to obtain resources throughout the state broken up by region. I wanted to know if I can create multiple sub-menu and include an attachment on the app. 

example.

1. First sub-menu (Northern, Central, Southern A, Southern B.
2. Second  sub-menu - resource type (Housing, Food, Clothing, Hygiene, ETC....)
3. On Resource example Housing. Will display name, number, website and pdf of flyer.

Would this be possible?
