The new released to GA Form submit automations are great except when they silently fail. I am facing a second time where the form submit automation no longer fires on form submissions. I have had a case open for >30 days when this happened last month as well and then suddenly started working again out of the blue. There are no error logs when this occurs, the Bot for the Form Submission event just doesn't trigger at all.
Is anyone else seeing this or have any ideas of what causes this? Google has been less than useless as I basically had to prove to them the last time that there were form submissions by granting access to the form results to an internal staff member. Since then, silence and only the canned responses of looking into this on a priority basis.

Did 30 days ago and the case is still open. Updated them yesterday as the issue recurred and still no updates. So support is not being super helpful even though we are on "Enterprise Plus" plan.

Create a app and test the form submission separately. If it fails you need to follow different method.
I have the same issue and is escalated already through AppSheet support. They will have to fix this using an enhancement release.
If my guess is right, you are using a Core Plan with the Core Security Settinfs turned on in the Google Admin console. This prevents external connections, and for some reason, AppSheet is seeing the Google Form submission trigger as an external connection. This is the same reason why the AppSheet events add-on for Google Sheet will not trigger the bots as well when changes are made to the Google Sheet.
If you try to build an app using a free plan, it will work because there is no Core Security setting applied on it.
AppSheet team needs to whitelist the Google Form and Events add-on as an internal connection.
I'm at least glad to hear this is impacting somebody else so maybe it will be taken seriously. But interestingly, in our case we have core security for Appsheet off as we do use external connections to a Cloud SQL database. So it's not being caused by that. For us the Form submission bot was rock solid until it wasn't and just stopped out of the blue with no errors in the logs or indications of any problems.
This had happened before, but then somehow after messing with the automation, it just started working again on a new deployment. I tried similar to what worked last time, but nothing seems to fix the bot this time. Probably better though as we have a workaround and at least it will be broken when support looks at it. Last time, they spent weeks trying to make us prove form submissions actually came in during the time the bot wasn't working.
I only wish our issue was a setting that I could turn off ๐

@nathanemg , I was on the support chat with Google on this issue a while ago. Following steps solved my issue.
This is apparently a bug with the feature and I hope they fix it soon. Hope this helps.
