So I have been trying to get this to work for some time, to no avail. So far the best advice I have been able to find is "Use Word". Essentially, when trying to generate a pdf using a template built in Google docs, its formatting comes out all broken. Here is what they look like.

Using Google Docs to generate the PDF is by far the best way - using built-in capabilities.
I don't understand the issue. The example PDF you have shown is literally exactly what you have implemented in the example template - as far as we can tell. What is the issue?
After looking at this again, I think you might be referring to the placement of the image and the column widths.
1) Image placement - I would use a document table. Insert a table with 2 columns and just a single row. Insert your image into the first cell. Then insert your text into the second cell. Make the borders white. By using a table, you can control the placement of those "header" items.
2) Column widths - if you do not specify a column width in your template table, the column will adjust width based on data content. In the Google Doc editor, set a static column width for each table column. The widths will carry over to the generated PDF.
