Can a PDF file exported with raw html have check boxes included

Hello Guys,

I'm generating a PDF file send as an email attachment which uses a HTML file with the following line of code but it seems to give an error which i don't see what is wrong.

This is the error i keep getting:

Try changing the <input> tag to be self closing <input/>
