Help with master-detail style form for patient admission

Hi folks, I’m starting my first app for a non-profit org that I support. We are digitizing our admission forms of patients. Each contains patient records such as name, date of birth. But we have several notes for each patient.

I have already created a form with Patient info, and there’s a deck list where we can search by name, and edit details.

Initially I thought on having notes on a 1-* association to each patient on the details window, but I could not find a way to create such link. Do I need to create a notes view and only access via a menu? Any samples on a similar use case?
