I want side by side columns in the form

I want to design my form (Questions) in the following format where an employee selects [ID] 1 and all related Sub ID are populated (as shown below A,B,C,D,etc) and against each [Sub ID], i want the employee to capture a [Comment]. Please suggest. Thank you.
