Prevent duplicate entry based on multiple columns

Hi, I want my app to provide an error or do a valid if check to prevent multiple entries.  There are 4 columns that I need to check to determine if a value is already entered.  It would be setup as columns "Date", "Person", "Location", "Game", "Score". 

I would assume that the Valid if would be done on the Score Column.  The valid if would check to see if all of the other 4 conditions were met based on the entries on the form compared to the Table that the collected values exist in.  Can someone help provide me an example of what that valid if would look like?
