Ref to Multiple Data Fields

Hi,

My Sheet 1 has this Data:

And my form is like:
Form VIEW

House Number: 890
Task: Wash Windows
Start Date: _________________
Actual Start Date: 03/01/2021
End Date: __________________
Actual End Date: 05/01/2021

What I want to know is how do I pull the Start & End Dates from my Sheet 1? The house number in my Form is a Ref in Sheet 1.
