Unable to update row in table 'X'. → Unable to save image → Request had insufficient authentic

I'm getting the below error when I attempt to upload an image using my app:

"Unable to update row in table 'X'. → Unable to save image → Request had insufficient authentication scopes."

I've tried the app with different app owner- and non-owner privileges along with different devices and have the same experience, i.e. error without the image uploading.

Troubleshooting suggestions are appreciated.

I am not certain but since the error concerns image security/authentication,  you might want to check the settings in the image below.  Does you app require secured image access?

Thanks for the reply.  Secure image access is disabled in the app while image signing is required since users will input personally identifiable information.

I've toggled each setting (though I understand the former would taken more time to become effective), without any change in results.
