You could do something like this using only Google Forms and Sheets. An administrator creates a Google Form called something like "UserA Feedback" for each user and attaches the responses to a Google Sheet, make sure that it is anonymous and the email address is not recorded. Then share the form to all those that are allowed to give feedback. Then give view only access to the results sheet to UserA.
