How do I remove third party apps from App Access Control?

It looks like I can set Trusted, Limited, Blocked, or specific OAth Scopes.  I need to remove, or 'forget' apps from the list.   My list is  very long and outdated, including every app ever granted and blocked access.  Some apps have multiple entries that appear to be duplicates.  I've downloaded them via csv, removed their entries and reuploaded, but that has no effect.

My only option is to block the unused ones.  The problem is that if a user requests a connection to a blocked one, I need to reauthorize it rather than just allowing it to be reestablished.
