Adding data to a slice by form

Hi,

I want users to add data to a slice by form.

I have created two views based on slices which are filtered by numbers (1,2) in the 'Number' column.
I am wondering how to add data by form to a slice not a table.

The number in "Number" must be the same as the one used in the slice. I have created this by using a dropdown list- the user has to select a number.

Do you know how to do this automatically?
Could you help me?
