Display name formula stopped working for some columns

Hi, I think that there is a bug. A formula that was working before has stopped working for some columns.

I have a series of columns that have a formula for Display Name.

The table has the following columns:
Key Result 1
Key Result 2
Key Result 3
KIWI 1
KIWI 2
KIWI 3

The display formula is:

Lookup
(

(Concatenate
(“Q4 (Oct - Dec)", “-”, USEREMAIL())
),

“Submission & Progress Q4”,
“Quarter-key”,
“Key Result 1”
)

This formula was working for all columns. But today I see the following problems:

1. Unable to pull the values from columns
   Key Result 2
   Key Result 3
   While it continues to work for column Key Result 1.

2. It is pulling the value of a column [KIWI 3] when it looks up [KIWI 2]. It is pulling the value from the following column on the table [What is important for you to do?], when looking up [KIWI 3]

I haven’t done any big changes on the tables and I’ve regenerated them just in case. Any clues at what may be happening?
