How create a button to open new forms?

Good evening everyone. I would like to know if you can help me.

1. I have a list of products, and I want that when I click on the Add icon a new form opens, where it brings me the name of that product and also a new box for me to put how many quantities of that product I need
2. After clicking on a send button, I can take it to my request window

I really appreciate your help guys.
