How to create an intake form?

Hello there. this is so basic question… but I really need some help!
I have an intake form for my clients, and I want to turn it into an app, but since there are so many questions in a single spreadsheet, I thought it might be a good idea to separate them into sections adding a couple more sheets … when I create the app with all the info at once, look decent, but when I tried to add several sheets I can’t make connected the info to the respective client … Can you please provide some advice?

If it looked good as a single sheet, why did you think it would be a good idea to separate them?

Generally, if you have things that are always 1-to-1 relations, I would default to keeping in all in one table.

If your issue is that your Form View seems way too long, you can use “Show” type columns to split it up into multiple tabs.



If you have records from different tables that need to be linked together, you need to use references.
