Image not show in generated pdf file.

Hello, 
I set a bot to create a PDF from a google doc template. In the google doc,  there will be a column for "image". However, the image won't appear but shows like this: 

I don't know why. 

And is it possible that I can fix the image in page 2 as the page 1 will be the place for my report. Currently the image will be forward to page 1 if my report is short.
