Strange behavior when uploading documents

I am observing a strange behavior in document loading that happens to me in several applications. Normally in Drive I have this structure for each application:

My_drive/Drive_APP_folder/Docs_folder.

The app is configured to save the documents inside that structure but suddenly it puts them directly into

My_drive/Docs_folder

And if it puts them there, the app then can't find them because another path is saved in the table.

I thought it was happening because some user was with an obsolete version of the app, I suspect it is not because of that. I see that sometimes, the same user, in the same session, generates documents inside the correct folder and also outside of it.
What can this behavior be due to?

Can someone help me with this. The application is saving files in both folders indistinctly and nonsensically. Sometimes it saves them in the correct one but sometimes not. what is going on?

Can you provide & check : 

1/ Your default app folder. Do you imply that you changed the default app folder ?

2/ The file column 'File path' parameter

Yes, I have checked that and it's ok the app has the direct path configured as

The file field has the following configuration

CONCATENATE('Medios_docs/',[id_elemen].[el_tipo],'_',[id_elemen].[identificador],'_',[id_elemen].[el_marca])

This generates one folder per vehicle, so all the vehicle documents are in the same folder.

Inside Medios_docs it looks like this.

The problem is that, at certain times, it creates Media_docs directly in My Drive. Even with vehicles that already have their own folder inside Medios_docs

Documents that are stored in Media_docs on my drive cannot be viewed from the app because the field content leads to Media_docs inside the app folder not to Media_docs on my drive.

I don't understand why, sometimes, it generates the folder tree outside.
I investigated if some users had obsolete versions of the application but it does not happen.
Sometimes it happens in the same user, in the same session and associating documents to the same vehicle. Something is happening that makes it generate the folder tree outside. It happens only in 10-20% of the documents.
