How to create a survey form with attachment?

When creating a survey form I want to attach a document PDF or word to be viewed with the form as an attachment

Along with the question you have an option to add a url.

you can upload this pdf to your drive and can set the view  permission for the required people, then add the url to your pdf along with the question.

Thanks for the answer. Anyway, that is one way of doing it,  I knew that already but, I was wondering if there is a way of just attaching a pdf or word or photo without putting in a link. ie. click attach voila!

@mggoogle  we already have the option to attach a photo as well.
