If I want to use an external database with Appsheet, and I have 500 total users of the app, do all 500 need Enterprise to allow the external db, or do only the application owner require Enterprise?

No, only the App Creator/Owner needs the Enterprise account. However, the type of account (Starter, Core, Enterprise, etc) determines which features are available to the app AND the price of each user license. Enterprise accounts allow access to external databases along with other features and as such each license comes at a higher cost.
Users of your app do not need to have an AppSheet account at all. They only need an email account within one of the authentication providers supported by AppSheet AND by your app - Google, Apple, Microsoft, Dropbox, etc.
